The stop follows, it never goes back to entry

Past the equivalent of its risk, a trailing stop takes over, at a distance from the high. It is not moved up to the entry price: that would turn a deliberate risk into a flat exit.

Tracking

The trade plan: the exit decided before the entry

Most losses come from an improvised exit. Once the position is recorded, the levels no longer depend on your mood that day: they move on their own, session after session.
